In Minneapolis, we source separate. I have a five bag system set up for all of my roommates to use! We also compost.

Buys Recycled - Do you purchase and/or use post-consumer recycled waste products, whenever possible?
Often these stats are hard to find on products, but if they are there, I am definitely more likely to purchase.
Just bought 100% post consumer recycled paper toilet paper!
Transportation - Do you use public transit, carpool, drive a hybrid and/or use non-motorized transportation?
I use my bike as much as possible. However, I still use a car for long trips, shopping, or if I have more to carry than can fit in my backpack.
Reusable Bags - Do you use reusable bags when shopping to avoid unnecessary plastic bag and paper bag production and waste?
I bring extra along when all the roommates go shopping!
Hibernate Computers - Do you set your computer monitor(s) to hibernate after 5 minutes and your hard drives to shut down after 30 minutes without use. (National authority on this recommendation: U.S. Energy Star)
Screen Saver - 3min
Screen Off - 5min
Hard drive shut down - 15min
To prevent water waste, do you limit your showers to 3 minutes or less and turn off the faucet when brushing your teeth?
This is something I am working on getting better at.
If anyone knows any good ways to work on cutting down, let me know!

Promotes Progress - Whenever possible, do you choose to support businesses and organizations that are transparent in their environmental actions and have published an eco-profile ... and have you sent requests to companies and organizations asking them to publish their eco-profile?
I love supporting local, green businesses! I have recently gone to getting my groceries primarily from the Seward Co-op. It's a little more expensive, but it tastes better and makes me feel better about what I eat.
Giving - Do you donate and/or volunteer for social and/or environmental betterment organizations?
This year I gave to the Sierra Club and Amnesty International.
I am the President of Green Biz, a club at the Carlson School of Management focusing on sustainability in the business community. We highlight green companies and companies trying to be more sustainable by bringing in guest speakers, going on company tours, and partnering with companies who are looking to reduce their or their customer's impact.
Lighting - Do you turn off lights when you leave the room?
I turn off the lights when I leave the room! Also, all the public space lights in my house have been converted to CFLs.
